How to Upgrade Items in Pokemon GO

Upgrade is one of the most prized items in Pokemon GO. It lets trainers transform Porygon into Porygon2 and allows them to grow in their Pokedex.

This item is rare but can be found by spinning a Photo Disk at a Pokestop or as a prize during special occasions. What can this item do?
